Health and Social Services Partnerships (HSCP) are the organisations created as part of the integration of health authority and counselling services in Scotland. Each partnership is managed jointly by the NHS and the local authority. So if you need the care of a district nurse and help with washing or preparing a meal, 1 professional will visit you and arrange the support you need. Two models of integration have been put to the choice of health and local authorities: the AHSCP offers a range of care and support services in the community for people with different care needs, as well as a social work service of the criminal justice system. The integration of health and social services is the Scottish Government`s reform agenda to improve services for people using health and adult social services. It ensures that healthcare and social services across Scotland are interconnected and transparent, particularly for people with long-term and often complex needs, many of whom are elderly or disabled. Under the AHSCP, doctors, hospitals, health workers, social workers and others work side by side. They exchange information and take a more coordinated approach to how services are delivered. The Shetland Islands Council and the NHS Shetland have agreed to formally delegate the health and welfare services of the adult community to a third body, the Shetland Integration Joint Board (IJB). The AIC is responsible for operational management and key decision-making for the Shetland Health and Social Services Partnership. Our vision is that by 2025, all members of their community will be supported to live longer and healthier lives, and we will reduce health inequalities and it is expected that every HSP will be able to improve care for the elderly by reducing delayed discharges, reducing unexpected hospital admissions and increasing the number of seniors cared for at home. In 2016, we passed legislation to consolidate health care and social services into one integrated system. Integration is the most significant change in health care and social services in Scotland since the NHS was founded in 1948. Integration aims to improve the care and support of people who use services, their caregivers and their families. To this end, more emphasis is placed on the consolidation of services and on prospective and preventive care. The Shetland Health and Social Care Partnership was established as part of the integration of services provided by the Shetland Islands Council and the NHS Shetland Health Board. The partnership manages health and community care services and provides a closer partnership between health care, social care and hospital services. These authorities are required to work with their local communities and care providers to ensure that care meets people`s needs. Drawing on a person`s skills, we will provide high-quality, person-centered care to improve their independence and well-being in their own community.
You don`t have to do anything else – the services worked together behind the scenes. Health and social workers work more closely together to support individuals and provide shared care in local communities in the Forth Valley. By working together, we can more easily identify those most at risk – for example, hospitalization. We can put together a combined set of care, support and lifestyle advice to keep you healthy and independent for longer. If you need hospital care, hospital staff work with community staff to help you leave with the appropriate support. Our total aggregate of additional direct investment in social protection and inclusion will be over £700 million between 2019 and 2020. The only area where a leading agency model was introduced was Highland, where health and social care budgets have been pooled since 2012.  The Public Bodies (Joint Work) Act 2014 (Scotland) provides the legal framework for the integration of healthcare and social services in Scotland.
National health and well-being outcomes are high-level statements about what the social and health partners are trying to achieve through integration and, ultimately, through further improvement in the quality of health care and social services. HSCP manages community health services and creates closer partnerships between health, social and hospital services. | HSCS Health and Social Care Scotland is a collaboration of health and social care leaders and managers from all health and social partnerships. The legislation created 31 integration agencies which are now responsible for £8.5 billion in funding for local services.
